Disability Support Worker
- Greater Sydney (South, West & Inner West)
Assist NDIS participants in Supported Independent Living (SIL) homes and during community access activities. Focus on personal care, meal preparation, travel training, and social connection.
Job Requirements:
- NDIS Worker Screening Check & Working with Children Check (WWCC)
- First Aid & CPR Certifications
- Australian Driver's License and reliable vehicle (highly regarded)
- Certificate III in Individual Support or similar (or equivalent experience)
Registered Nurse (RN) - Community Care
- Sydney Metro (Head Office in Yagoona)
Oversee and deliver clinical community nursing supports. Manage medication plans, PEG feeding, complex wound care, and conduct health safety training for support staff.
Job Requirements:
- Current AHPRA registration as a Registered Nurse
- Minimum 2 years of community health or NDIS clinical care experience
- Strong understanding of NDIS high-intensity support guidelines
- Exceptional document reporting and communication skills
Intake & Care Coordinator
- Yagoona Office
Coordinate intake processes for new participants, manage roster allocations for compatibility, compile NDIS RoC (Roster of Care) submissions, and liaise with Support Coordinators.
Job Requirements:
- Prior experience in NDIS care coordination or administrative intake
- Exceptional organizational skills and database management
- Deep understanding of NDIS pricing limits and funding structures
- Patient, empathetic phone manner and relationship-building skills
